Tax Return Preparation Checklist

Now is a good time to begin thinking about all the information you need to collect before the preparation of your tax return. The list below lists a lot of the usual items. Not all of these items may apply to you and you may have additional information that needs to be evaluated.


Personal Information:


  • Name, address, date of birth and social security number or ITIN of you, your spouse, and your dependents

  • Driver's license or another form of identification

  • Copy of the most recent tax return

  • Bank account and routing number if you want your refund deposited directly into your account


Wage or Income Statements:


  • W-2 - Wage and Tax Statement

  • 1099-Misc - Miscellaneous Income

  • 1099-NEC - Nonemployee compensation

  • 1099-G – Unemployment compensation

  • 1099-C – Cancellation of Debt

  • 1099-R - Distributions from Pensions, Annuities, Retirement

  • 1099-S - Proceeds from Real Estate Transactions

  • 1099-INT – Interest Income

  • 1099-DIV – Dividend Income

  • 1099-B - Proceeds from Broker and Barter Exchange Transactions

  • 1099-OID - Original Issue Discount Income

  • 1099-SA – Distributions made from HSA, Archer MSA or Medicare Advantage MSA

  • K-1 - Partner’s Share of Current Year Income, Deductions, Credits, and Other Items

  • SSA-1099 - benefits received from Social Security

  • Alimony received

  • Rental property revenue


Expenses, Deductions, and other Itemized Items:


  • 1098 – Mortgage Interest Statement

  • 1098-E – Student loan interest paid

  • 1098-T – Tuition paid

  • IRA contributions

  • Childcare expenses and providers information

  • Self-employment related expenses

  • Gifts to charity

  • Investment and tax preparation expenses

  • Casualty/theft losses

  • Medical and dental expenses

  • Alimony paid

  • Rental property related expenses


Taxes and Other Payments:


  • State and local income taxes

  • Real estate taxes

  • Estimated taxes
